Strategic Guidance & Support
Coordinating Committee
Core Actions
  • Engage in development of TtZ 2025/2026 Priorities & Actions resource.
  • Provide leadership and insight to service mapping processes and Round Tables.
  • Develop specific Terms of Reference to strengthen clarity of roles and accountabilities.
  • Create a process for establishing permanent Coordinating Committee and Co-Chair roles.

Expected meeting frequency

  • Approximately monthly
Ambassador Table
Core Actions
  • Intentional and strategic Information sharing.
  • Receive additional updates on ongoing TtZ initiatives via email.

Expected meeting frequency

  • 3 – 4 mtgs/year
OHTN – Backbone Infrastructure Support
Core Actions
  • Provide coordination support for all TtZ groups.
  • Facilitate service mapping exercises.
  • ‘Setting up the Fast Track’
    o   TtZ website
    o   Fast-Track Cities global dashboard
    o   Internal project mgt. processes
    o   TtZ social media
  • Monitor and review recently completed US modules, contributing to, leveraging and incorporating Canadian efforts.

Ongoing Support

  • Work with any Liaison roles that may arise to monitor, contribute and, where alignment exists, incorporate, to leverage within TtZ.
  • Create partnerships across the sector to facilitate support for low-income seniors living with HIV.
  • Provide support to HQ and Blue Door Clinic.
  • Leverage provincial campaigns to promote Toronto-specific messages.
  • Continue to engage in efforts to improve access to HIV medications including PrEP, PEP, and ART on a provincial level.

Testing Action Team


Launch innovative testing programs and improve linkage to prevention, treatment and care. [TtZ Plan, Step 1]

Core Actions
  • Implement any identified testing-related initiatives/actions arising from connection to care systems mapping (particularly newcomer and complex care lens) and Round Table discussions.
  • Prioritize HIV testing recommendations from the TtZ Plan that are specific to Toronto.

Ongoing Support

Expected meeting frequency:

  • As required (complementing provincial bi-monthly testing meetings)
Newcomer Action Team

Respond to newcomer support theme recommendations from the TtZ Plan, current Toronto data on new infections and the unique barriers experienced by newcomers.
Core Actions
  • Map Toronto care cascade service pathways for newcomers including linkage to care and clinical service pathways.
  • Host a Round Table to:
    o    review results of service mapping project, assessing against TtZ Plan.
    o    engage in collective analysis and identifying key gaps/initiatives for action.
  • Implement identified initiatives/actions arising from Round Table discussions. [Note that updates will be made in this section following the Round Table]

People with complex needs Action Team

Provide specialized care, intensive case management, and supportive housing programs for people with complex needs (i.e. mental health challenges, substance use, unstable housing, experience with the justice system). [Step 2, #6]
Core Actions
  • Map Toronto care cascade pathways for people with complex needs.
  • Host a Round Table to:
    o   Review results of service mapping project, assessing against TtZ Plan recommendations.
    o   Engage in collective analysis and identify key gaps/initiatives for action.
  • Implement identified initiatives/actions arising from Round Table discussions. [Note that updates will be made in this section following the Round Table]
Advisory Circles
Data Advisory Circle Ongoing Support
  • Champion a Toronto-specific lens to provincial efforts towards improving data collection.
  • Provide expertise in data collection practices for ongoing TtZ initiatives and groups.
